I've been to Form Spa for surgery, botox, and filler. I have been treated with courtesy, respect, and caring every time I've gone. I’m impressed and awed by the skill of Dr. Ward and his staff. 10/10 would recommend!2020 UPDATE: I’ve also had several hydrafacials, brow tint/wax, and lash lift and tint, and I’m beyond satisfied!! I look so good after!2022 UPDATE: I love getting lash lifts and tints with Daphne! She’s really good at her job, obviously, but she’s also so fun to talk with!

I have not yet had a treatment but I wanted to give a big thank you to Lisa, who I was fortunate to reach when I called. She was kind, sweet, helpful, and authentic. She helped me to figure out the treatment I wanted, gave me some info, asked a colleague for additional info, and got me scheduled. This sounds pretty basic but it is not always the norm. I so appreciate it and know that my actual treatment will be great based upon this initial experience. I will post then too. TY!!!

I love Form Spa! My skin has dramatically changed for the better. Acne scars and pigmentation has decreased significantly! I definitely recommend the Pico Resolve Laser treatment and all the products! My favorites are the vitamin C serum and face wash!

Love Form Spa... My estitician Daphne is excellent. They are full service... From facials to injections to cosmetic surgery. Beautiful interior, friendly, inviting. Also, the owner/MD has own line of skincare and makeup. Highly recommend Form Spa.

This place is like no other in this valley. Upon arrival you will be greeted in the reception area by very friendly and helpful staff. The spa is beautifully decorated and comfortable, definitely a place you could relax in. The medical staff, the one and only Dr. Ward…. Excellent!

Daphne is absolutely amazing!!! I had my first hydro facial with her and my skin is so happy and glowy. Daphne is so knowledgeable and professional along with so warm and friendly. She made sure I knew what to expect and answered any question I had! The spa is warm and welcoming, beautiful and great energy from everyone I interacted with. I left feeling relaxed and refreshed. I can’t wait for my next visit. Thank you Daphne and everyone for making it such a great experience 💛

Form Spa has wonderful staff and services. They take the time to explain each step that they are taking and make sure that you are comfortable. It is a beautiful space and the professional services they offer are outstanding. I can’t wait to return and be beautified by Heather and Daphne again!
